Prescription Refill Request


Premier Medical policy is that we do not prescribe new medications, antibiotics or certain other medications without seeing the physician. This refill request is to be used for routine maintenance medications.

On weekdays, we will respond to your request within 48 hours. If you are submitting your request Friday after 1:00 pm, Saturday, or Sunday, your prescription may not be ready until Tuesday. If you need the refill immediately, please call our office. Upon receipt of the following form, we will ask the doctor to review your request.
